There are 3 ways to register:
1)Download, Print and Snail Mail Application (send cheque for payment)
DOWNLOAD the application forms, printthe forms, complete the forms following all instructions and use Canada Post to mail your application, proof of vaccinations and cheque.
2) Download, Scan and Email Application (use PayPal for payment)

. After emailing your application forms, submit your payment online by credit card using PayPal. *Please note that an additional service fee applies for selecting this option.* You will be sent an email to confirm that we have received proof of payment and to confirm your registration.
3) Request an Application be Mailed to you (send cheque for payment)
Unable to download the application forms - no problem! Call our answering machine 416-233-7633 and request that the forms be snail mailed to you. When leaving your mailing information please speak slowly and clearly providing your name (including spelling), street name (including spelling), city, postal code and phone number. When you receive the forms, complete the forms following all instructions and use Canada Post to mail your application, proof of vaccinations and cheque.
